Epiphany Care Homes was founded by Aimee and Derek Slaughter as an alternative to traditional large-scale assisted living, memory care and nursing home facilities.

Aimee is a dedicated registered nurse with over 22 years of experience in the community setting. Derek is retired military and has an exceptional background in construction. They've formed a team of friendly professionals to provide amazing personalized care and skilled nursing services to individuals in the Minnetonka, MN area.
